Patient reports acute onset low back pain 5 days ago after lifting a box at work, no radiation, no numbness or tingling, no bowel or bladder changes, no fever, no history of cancer, no trauma beyond the lifting incident. Denies any prior back pain episodes. Has not yet tried NSAIDs, activity modification, or physical therapy. Exam shows mild paraspinal tenderness, full strength and sensation, negative straight leg raise. Requesting MRI immediately per patient request.
